MILLIONS of pounds of funding for schemes designed to help people remain independent in their own homes and to speed up discharges from hospital is set to be awarded across Wales shortly.
And some of it could be heading for Gwent as part of the first round of investments to be made from the Welsh Government’s new Intermediate Care Fund.
The fund comprises £50m; £35m of revenue funding in the local government budget and £15m of capital funding in the housing and regeneration budget.
The aim is to use it to drive collaborative working between social services, health and housing, alongside the voluntary and independent care sectors, to help avoid unnecessary hospital admissions, or inappropriate admission to residential care, as well as ensuring people can be discharged from hospital back home, when appropriate, as soon as they are well enough.
Frail and elderly people, and those with complex health and social care needs are the target group.
