Gwent Police have said two people were taken to hospital following the crash - both with "potentially life-changing injuries".
A spokeswoman for the force said there were two people in each car involved in the crash.
She said: "It appears two people were taken to hospital - a 84-year-old man, who was initially trapped and removed from the vehicle with the assistance of the fire service.
"The other was a 77-year-old woman. Both have serious, potentially life changing injuries."
The spokeswoman said the crash was reported to them at 1.05pm and that the road was re-opened at 3.44pm hours, after the scene was examined and debris was removed by the council.
UPDATE 4.30pm
The road has now reopened.
Ebbw Vale Officers tweeted "Multi agency working at its best following a road traffic collision on the A4046 Waunllwyd earlier today. Our thoughts are with all involved."
UPDATE 2.45pm
A spokeswoman for South Wales Fire and Rescue Service said a person has been taken to hospital following the crash.
She said they attended the crash at 1.05pm to help get the person out of a car.
The road remains closed and there is very slow traffic in the area.
2.15pm
A ROAD is blocked in both directions following a two vehicle crash.
The crash in the A4046 with Cemetery Road is affecting traffic between Cwm and Ebbw Vale.
